To register with the Practice patients are required to complete a patient registration form, which is available at the Reception desks. A sketch map of the Practice area is available on the link Catchment Area. Only patients who live within the Practice area may join the Practice. New patients will be asked to have a health check by one of the Practice Nurses.
When you register with the Practice you will be asked if you have a preference for which doctor you wish usually to see.
